Why surgery matters
Understanding Your Options
Management, not a Cure — but Significant Relief is Possible
If left untreated, lipedema progressively diminishes the softness of your skin, your mobility, your self-esteem, and your quality of life. Over time, it increases the risk of blood clots, lipo-lymphedema, skin ulcers, infections, and severely blocked lymphatics.
Dr. Schwartz will recommend treatment based on your needs and stage of lipedema. The goal is not cosmetic perfection — it is functional improvement, pain reduction, and halting of disease progression. Treatment falls into two categories: conservative management and surgical intervention.
Conservative Management
Reduces symptoms and slows progression — but does not remove lipedemic fat or halt the disease permanently.
- Medical-grade compression garments
- Manual lymphatic drainage (MLD) massage
- Specialized low-impact exercise (swimming, cycling)
- Anti-inflammatory dietary approaches
- Pain management and physical therapy
Surgical Intervention
Removes diseased fat tissue, halts progression, and provides lasting improvement in pain, mobility, and quality of life.
- Lymphatic Sparing Liposuction (tumescent technique)
- Manual Lipedema Extraction (MLE) — Dr. Schwartz’s specialized technique
- Skin excisions for patients with excess skin after volume reduction
- Panniculectomy for abdominal skin overhang
- Lymphatic sparing arm and thigh lifts
Non-Surgical Options
Conservative Management: What Helps and What Doesn’t
Conservative treatments are an important part of long-term lipedema management — they reduce symptoms, maintain mobility, and help prevent rapid progression. However, they do not remove lipedemic fat, and their effectiveness decreases as the disease advances through the stages.
Compression Garments
Medical-grade compression reduces fluid accumulation and provides structural support for affected limbs. Must be fitted correctly — poor fit due to disproportionate fat distribution is itself a complication of advanced lipedema.
Manual Lymphatic Drainage
Specialized massage technique that stimulates lymphatic flow and reduces fluid buildup. Performed by a certified lymphedema therapist — not standard massage. Particularly effective in early-stage lipedema.
Specialized Exercise
Low-impact activities like swimming, aqua aerobics, and cycling support lymphatic function and overall cardiovascular health without placing excessive load on affected joints. Walking is encouraged but will not reduce lipedemic fat volume.
Dietary Approaches
Anti-inflammatory diets (Mediterranean, low-glycemic) may reduce systemic inflammation and support overall health. Diet alone will not reduce lipedemic fat — but it may help slow symptom progression.
Surgical Intervention
Dr. Schwartz’s Surgical Treatment Approach
Dr. Schwartz’s surgical plans are built around two categories of treatment: lipedema reduction surgery to remove diseased fat and restore function, and skin excisions for patients whose skin has lost elasticity over the course of the disease. All procedures use lymphatic-sparing techniques to protect the lymphatic system throughout surgery.
Lipedema Reduction Surgery
Lymphatic-sparing procedures that reduce painful, nodular lipedema fat to alleviate localized symptoms, restore functional mobility, and prevent further disease progression.
Skin Excisions (Including Lipectomy)
Why Specialist Care Matters
Why Standard Doctors Often Get it Wrong
Lipedema treatment is not about fixing a cosmetic problem
This is why you need fat disorder experts and plastic surgeons well-versed in lipedema. Dr. Schwartz and the teams in Beverly Hills and Arizona understand that lipedema treatment is not about fixing a cosmetic problem — it is about improving your quality of life with targeted medical intervention.
Coverage & Costs
Does Insurance Cover Lipedema Surgery?
Coverage for lipedema surgery varies significantly by insurance carrier. At TLC, our dedicated insurance team pursues prior authorization for medically necessary lipedema surgery on your behalf — navigating a 3-part process: Prior Authorization, GAP (in-network request), and Single Case Agreement.
Not all patients qualify for coverage, and every insurance carrier establishes its own clinical benchmarks. For patients who proceed on a cash basis, reimbursement submission is still possible after the procedure when surgical codes are deemed medically necessary.

Our insurance team handles the process for you.
